may do so by mailing in a donation form and a check made out to Choice & Consequence. Please
be sure to include all of your contact information and a phone number so
that we can send you a thank you letter, which you can use for income tax
purposes. We would like to thank you in advance for your support.

Your donation will be
spent providing direct services to youth in schools. About 20% goes to
indirect services such as, accountant, legal, taxes, etc.

student in a school is $2-$4.
